Riggs and Murtaugh respond to the murder of an escaped patient of Doctor Cahill's who had mysteriously lost their mind, but the investigation brings a hidden scheme to light that is far bigger than they imagined. Meanwhile, Riggs' anger gets the better of him once again, causing Avery to consider whether or not he is fit to serve, and family tensions run high between Murtaugh and Trish, who need to make some big decisions.
