On a day like any other in Northern Italy, Simone finds himself, as he often does, kayaking down a beautiful river and taking in the quiet evening. Then suddenly Simone's tranquility is interrupted by the sound of a car crash. A renowned surgeon, Simone rushes to the scene to offer assistance, and when he arrives he sees a gravely injured man bleeding profusely. Quick to action, Simone calls for an ambulance and makes a tourniquet out of a seatbelt to help stop the bleeding. And then Simone sees the man's tattoo: a sizable swastika in the center of his chest. As the son of Holocaust survivors, Simone is faced with an impossible choice: save this neo-Nazi's life or to let him die. The film follows Simone in the aftermath of the accident as he deals with the consequences of his decision.
