Godine 1838. u zabačenoj provinciji na jugu Italije, bandita Đuzepea Karuza ubija Malakarnea, lovočuvar na imanju barona Sanđermana. Devet mjeseci kasnije, njegova žena Maria, rađa sina Đovanija. U isto vrijeme se, u kući barona, rađa Kostanca, čija majka umire na porođaju. Baronu će biti potreba dojilja za bebu, a sudbina je namjestila da to bude Maria.
